Please send referrals and forms for potential Home/Hospital students to the referral mailbox: hhinfo@rcsdk12.org or fax Home/Hospital Nurse Christine Proctor at (585) 324-9931.
Maternity students must be referred by a Medical Doctor (MD) to be placed in the Home/Hospital Program. The Medical Doctor (MD) note requires the Estimated Date of Delivery (EDD) verification or delivery information, and must be signed by a Medical Doctor (MD). These documents must come from the Medical Doctor (MD), hospital, or clinic that is caring for the student.

Guidance counselors in schools can also contact their school nursing department with medical requests for the Home/Hospital Program. School nurses will contact the Home/Hospital Nurse Christine Proctor at (585) 254-1240 ext. 3432 or Chris.Proctor@rcsdk12.org
A signed HIPAA must be completed and faxed to Home/Hospital Nurse Christine Proctor at (585) 324-9931. Christine Proctor can be reached at (585) 254-1240 ext. 3432 or Chris.Proctor@rcsdk12.org to ensure receipt.
The Health Insurance Portability and Accountability Act (HIPAA) is a federal law that required the creation of national standards to protect sensitive patient health information from being disclosed without the patient's consent or knowledge.
Prospective students are placed on a maternity list with their due dates. An intake is done by the Home/Hospital Assignment Coordinator Michele Mazzola or the Home/Hospital Nurse Christine Proctor. Students start with Home/Hospital Instruction on their due date or the date closest to their delivery date.
Period for Home/Hospital Instruction:
6 weeks if delivery is normal vaginal delivery (NVD)
8 weeks if delivery is Caesarian Section (C/S)
Home/Hospital Instruction can be extended for delivery complications, infant illness, or hospitalization.
Home/Hospital Instruction is not extended for lack of daycare or childcare.
