In order for your student to participate in Jaguar Athletics, registration is mandatory.

We’re excited to welcome your child to the team—but first, let’s make sure they’re officially signed up and ready to compete.

This form must be completed by a parent or legal guardian. 

🚫 Important: 🚫

Students who are not registered through ArbiterSports will NOT be allowed to try out, practice, or compete in any sport.

Please complete the registration before the start of each sports season to ensure your student’s eligibility.

🩺 Medical Physical Requirement 

All athletes must have an up-to-date medical physical on file. 

If your child does not have a current physical, no problem—they can receive one at school through our on-site Rochester Regional Health Center.
